There are times when things just don’t go right, correct?  And you look at it and sometimes you try to want to control the situation and sometimes we don’t control the situation the best way. So here is a solution and it’s a solution that works for us and it works nearly every time. You know what that is? It’s go back a step.  Look at where things were going right and then when did things go wrong and follow the sequential steps that occurred in that process.  Go back a step. Maybe you’ll have to go back two steps, but go back one step and see what occurred and that’s where our investigation begins. When we go back a step we can see all of the right things that were done and then we can see where things may have gone wrong and at its best you can institute a policy to make sure that doesn’t happen again.
